Forms and Reports
This section has the forms and documentation which are critical to the school’s ability to comply with accrediting standards that schools need to submit to inform the Commission of any changes which affect the following facets:
• Ownership
• Programs
• Educational purpose
• Management of the school
• Administrative changes
A school is responsible through its reports for demonstrating continuing compliance with accrediting standards. A school is responsible for monitoring when appropriate applications and reports need to be filed. Applications and reports submitted for Commission review must be complete, accurate, signed by the school’s chief executive officer, and filed in a timely manner.
Applicable fees must accompany the application or report. At its discretion, the Commission may prescribe a specific time frame that is different from the customary period for the submission of school applications or reports. Click here (Forms and Reports) to access IBPCA's forms and reports.
